DATA SUBJECT APPLICATION FORM AS PER LAW ON THE PROTECTION OF PERSONAL DATA NUMBERED 6698
In Law on the Protection of Personal Data numbered 6698 (referred to as “LPPD"), personal data owners (referred to as “Applicant"), defined as “data subject", are granted the following rights under Article 11 of LPPD:
- Learning whether your personal data is processed,
- Requesting information if your personal data has been processed,
- Learning the purpose of processing your personal data and whether they are used appropriately,
- Knowing the third parties to whom your personal data is transferred at home or abroad,
- In case your personal data is incomplete or incorrectly processed, requesting that it be corrected and requesting that the transaction carried out within this scope be notified to third parties to whom your personal data has been transferred,
- Requesting the deletion or elimination of your personal data and notifying the third parties to whom the personal data has been transferred, in the event that the reasons requiring processing are no longer valid, although it has been processed in accordance with the provisions of other laws and other legislation related to LPPD,
- Objecting to the occurrence of a result against you by analyzing the processed data exclusively through automated systems,
- Requesting compensation in case you suffer due to unlawful processing of your personal data.
Pursuant to Article 13 paragraph 1 of the LPPD and the the Communique on Procedures and Principles of the Application to Data Controllers, the applications to be made to our Company regarding these rights shall be made in writing or by using the e-mail address previously notified to our Company and registered in our Company's system shop.moltr@molgroup.com to send an e-mail to the address or by other means to be determined in the future by the Personal Data Protection Board (referred to as the “Board".).
Within this framework, the applications to be made to our Company in “written” form, will be available to delivery,
· By applying to the Company personally by the Applicant,
· By notary or by registered and reply paid letter,
· By sending an e-mail to our Company's shop.moltr@molgroup.com address, in case the Applicant has an e-mail address they can use which is registered in the Company's systems,
with the print out of this form.

In accordance with Article 13, paragraph 2 of the LPPD, your applications submitted to us will be replied within thirty days from the date on which your request is received by us, depending on the nature of the request. In accordance with the provisions of Article 13 of the LPPD, our responses will be delivered to you in writing or electronically.

This Data Subject Application Form has been prepared in order to be able to respond to your application correctly and in the prescribed term by identifying your relationship with our Company and determining your personal data processed by our Company in full, if available. Our Company reserves the right to request additional documents and information (copy of identity card or driver's license, etc.) for identification and authorization in order to eliminate the legal risks that may arise from unlawful and unfair data sharing and especially to ensure the security of your personal data. In the event that the information regarding your requests submitted with the Data Subject Application Form is not accurate and up to date or an unauthorized application is made, our Company does not accept responsibility for the requests arising from the said inaccurate information or unauthorized application.
